Eyelid and Facial Spasms (Blepharospasm)
This is a condition where there is frequent blinking or involuntary spasms of either both sides or one side of the face. Eyelid and facial spasms can cause loss of vision because of the increased blink rate, eye irritations, and eyebrow spasms. Frequently, Botox is used to treat abnormal spasms.
Eyelid Lesions
Various “lumps” and “bumps” can be found on and around the eyelids, eyebrows, and face. Consultation is needed to determine if this requires removal, partial removal, or biopsy or if it can be observed. When removal or biopsy is needed the specimen will be sent to the laboratory to determine if the lesion is benign or malignant.
Benign lesions can usually be observed or simply removed. Malignant lesions usually require additional treatment, which will be discussed with the patient in detail.
The majority of the eyelid, eyebrow and facial lesions can be removed or biopsied in an office setting. Most office-based excisions can be performed without anesthesia, stitches, or patches.
Eyelid Malpositions
This would include eyelids that turn out (ectropion) or turn in (entropion) or lid malpositions that create asymmetry.
Ingrown Eyelashes (Trichiasis)
This condition can be either from lashes growing in the wrong direction or from the eyelids being in a poor position. This can cause chronic eye irritation, discharge, and possibly infection. Treatment can include permanent removal of abnormal eyelashes or correction of the eyelid malposition.
Orbital Tumors
The orbit is the bony socket that contains the eyeball. Orbital tumors are abnormalities within the bony socket, but outside of the eye.
Thyroid-Related Eye Problems
Patients can have hyper or hypo thyroid conditions which can cause swelling of the eyelids and bulging of the eyes. Eyelid retraction and prominent eyes can cause a variety of both functional and cosmetic issues.
Tearing Problems
Both children and adults can have excess tearing in one or both eyes. The causes may include both eyelids and problems with the tear duct drainage system. Treatment may involve non-surgical and surgical solutions.
Upper Eyelid Functional Blepharoplasty
The Upper Eyelid Functional Blepharoplasty removes excess skin that is causing peripheral vision loss.
Reconstructive Procedures
Loss or Deformity of the Globe (or Eyeball)
In cases where there is a loss of vision and a deformity of the globe, or chronic eye pain, this can necessitate the removal of the globe or globe contents. Considerations for both function and cosmetics are important considerations to discuss with the patient. Procedures in this category could include enucleation, evisceration, and ocular and orbital implants.
Scarring (Retraction)
There are many causes of eyelid scarring including prior cancer removal or treatment, trauma, burn, and from prior surgery. The result can cause an abnormal position of the eyelids, eyebrows, and periorbital structure. This can result in reduced or loss of vision, tearing, eye irritation, and eyelash touch to the eye.
Trauma
Trauma can result in injuries to the globe (eyeball) or the surrounding facial structures including the bones and soft tissue. Treatment could include fracture repair and or soft tissue repair and reconstruction.
Tumors
When a lid mass is determined to be cancerous, most are treated by complete removal and repair. Some can be removed and repaired in one procedure. Other patients need a more comprehensive approach which could include the “Mohs” technique or a hospital-based multidisciplinary clinic.
Benefits of Oculofacial Procedures
Oculofacial procedures offer a multitude of benefits, both aesthetic and functional.
Aesthetic benefits include the rejuvenation and enhancement of the eyes and surrounding facial features. Eyelid surgery (blepharoplasty) can address issues such as drooping eyelids, puffiness, and wrinkles, providing a more youthful appearance. Brow lifts contribute to a refreshed and lifted look, reducing forehead lines and creating a harmonious balance.
Beyond aesthetics, these procedures can have functional advantages, such as improving peripheral vision obstructed by sagging eyelids.
Oculofacial procedures also improve overall well-being. Patients frequently report feeling more energized and satisfied with their appearance, translating into positive impacts on their social and professional lives.
Ideal Candidates for Oculofacial Procedures
Determining ideal candidates involves a comprehensive assessment of individual goals, health, and expectations. Individuals who complain about sagging eyelids, excess skin, or brow descent may come to us for a consultation. Additionally, those experiencing functional issues like impaired vision due to drooping eyelids may benefit significantly from these procedures.
Candidates should be in good overall health, with realistic expectations about the outcomes. The consultation process plays a crucial role in identifying and discussing candidacy, as surgeons can evaluate specific concerns and tailor recommendations accordingly.
The Consultation Process
The consultation is a crucial step in the oculofacial procedure journey. During this session, the surgeon and patient engage in open communication to establish goals, discuss concerns, and explore potential procedures. Surgeons assess the patient’s medical history, current health status, and specific anatomical considerations related to the eyes and face.
Patients have the opportunity to ask questions, gain insights into the recommended procedures, and understand the potential risks and benefits. This interactive process helps build trust and ensures that the chosen procedures align with the patient’s expectations and desired outcomes.
How to Prepare for Oculofacial Surgery
Preparation for oculofacial surgery involves both physical and mental readiness. Patients receive detailed pre-operative instructions, including guidelines on medications, lifestyle adjustments, and dietary considerations. Quitting smoking and avoiding certain medications that may increase the risk of bleeding are common preparatory measures.
Mentally, patients should be prepared for the recovery process and have realistic expectations regarding the results. Surgeons often encourage open communication during this phase to address any lingering concerns and ensure a positive mindset heading into the procedure.
Oculofacial Surgery Recovery
Recovery from oculofacial surgery is a vital aspect of the overall process. Post-operative care instructions are provided to guide patients through the initial healing period. This may include information on managing discomfort, caring for incisions, and avoiding activities that could strain the surgical site.
Follow-up appointments allow the surgeon to monitor progress, address any concerns, and provide further guidance on gradually resuming normal activities. The recovery timeline varies based on the specific procedure performed, but most patients experience gradual improvement in both function and appearance as they progress through the recovery phases. Patients should adhere to post-operative instructions that contribute significantly to the success of the recovery process.
